Required Outcomes and Benchmarks This telehealth expansion initiative aims to improve healthcare access and utilization for underserved populations in rural Illinois. The key outcomes include: 1) Increasing the number of patients receiving remote consultations by 25% within the first year, 2) Achieving a patient satisfaction rate of 85% or higher for telehealth services, and 3) Reducing the average travel time for patients to access care by at least 30 minutes.

Key Performance Indicators (KPIs) to Track To measure the success of this program, grantees must closely monitor the following KPIs:
