Accessing Telehealth Services for Underserved Populations
Required Outcomes and Benchmarks This telehealth expansion initiative aims to improve healthcare access and utilization for underserved populations in rural Illinois. The key outcomes include: 1) Increasing the number of patients receiving remote consultations by 25% within the first year, 2) Achieving a patient satisfaction rate of 85% or higher for telehealth services, and 3) Reducing the average travel time for patients to access care by at least 30 minutes.
Key Performance Indicators (KPIs) to Track To measure the success of this program, grantees must closely monitor the following KPIs:
- Number of telehealth visits conducted per month
- Percentage of patients utilizing telehealth services who previously lacked access to in-person care
- Average wait time for telehealth appointment scheduling
- Percentage of patients reporting high satisfaction with the quality of telehealth services
- Reduction in missed appointments due to transportation barriers
Evaluation and Reporting Requirements Grant recipients are required to submit quarterly progress reports detailing their performance against the stated outcomes and KPIs. Additionally, they must participate in an annual evaluation process, which may include site visits, data audits, and stakeholder interviews. Continued funding is contingent upon demonstrating measurable improvements in healthcare access and utilization for the target population.
Performance Thresholds and Disqualification Failure to meet the minimum performance thresholds, such as a 15% increase in telehealth visits or a patient satisfaction rate below 75%, may result in partial or complete funding reductions. Grantees may also be disqualified if they fail to establish partnerships with local health providers, implement culturally competent care practices, or comply with HIPAA and other regulatory requirements.
